Please bring the following items with you on your first visit:

Your health history and any allergies.
Any surgical reports, notes from your referring doctor.
Any laboratory and/or imaging reports.
All your medications or written on a list.
Your ID card and personal health insurance card.
Your PPI number if you have applied.
Any questions you may have (writing these down).
A friend or family member.
